Yeah, that’s not very typical, I’d like to make that point.
Many magazines have removable retention plates so you can replace the magazine spring. However, the design of most magazines is such that the retention plate is held into place by a lot of material and the retention plate needs to be slid like an inch and a half or more before it can be removed and you have to push pretty hard because the magazine spring is pushing perpendicular on the retention plate, especially when fully loaded.
When the retention plate is out of position it is really noticeable because it sticks out.
My guess is that he didn't install the retention plate wrong or knock it out by accident and he likely suffered a material failure. As with all material failures, it is low probability but not impossible from manufacturing errors or wear and tear.

Maybe the ammo was a too hot loaded, over pressure (not extreme overpressure) will bang out the mag (and destroy it) would also explain why he has problems reloading
